Application
Community services: Referrals will be accepted from a person requiring service, physicians, family members, friends, spiritual and religious leaders and health care professionals * people being referred to the program must have approval from the Power of Attorney and accept the services based on the established criteria * admission is based upon medical, physical, emotional and spiritual need

People living with the challenges of life-limiting illness and their families * anyone who has suffered through grief and loss (bereavement) or spiritual distress
