Description of Services:

Provides 6 to 8 weeks of support to older adults (50+) with developmental disability who are either residing in or preparing to move into long term care facilities

staff will plan and develop a transition plan to assist a person to move seamlessly from life in the community to life in a long term care home
the goal is to help people maintain their connections with community involvement and participation
