Description of Services:
24-hour pre-scheduled appointments or urgent on-call response for support services in the client's home to help with personal care and with essential homemaking * multiple daily visits based on individual needs, available 365 days a year, provided by trained personal support workers (PSW) * personal care could include bathing, dressing, grooming, medication assistance/reminders, light food preparation, etc, based on needs assessment * safety reassurance checks
