Description of Services:
Assessment and treatment of older adults residing in the community who are identified as being at risk of further decline in daily function performance * screening for other health-risk indicators is also conducted * recommendations are made to clients, families/caregivers, and referring health partners as part of the healthy aging focus of the clinic
Services offered:
Standardized Comprehensive Geriatric Assessment (CGA) and consultation by a geriatrician, physiotherapy and occupational therapy services available. Speech-language pathology, registered dietitian and social worker on a consultative basis
