Description of Services:

Community-based case management service for Mississauga patients (excluding Malton) and those in Etobicoke (Southwest), with a history of serious and persistent mental illness, often a psychotic disorder * case managers work in partnership with clients to develop the skills and supports they need to live successfully in the community * meet with clients in their own home or in the community * focus is on goal based activity and community integration that supports a client’s recovery and eventual discharge
