Description of Services:

Provides community-based support for individuals who are homeless or at imminent risk of becoming homeless and are diagnosed with a serious and persistent mental illness * Trillium Health Partners is in partnership with community based organizations which provides housing and support

Services include:

case management
home visits
supportive counselling
rehabilitation assistance

Following assessment and acceptance by the program, the client is assigned a HASP worker from one of the agencies:

Centre for Addiction and Mental Health (CAMH)
Indus Community Services - Mississauga and Brampton
Peace Ranch
Peel Addiction Assessment and Referral Centre (PAARC)
Supportive Housing in Peel (SHIP)
