Description of Services:
Hospital-based outpatient clinics and programs with a focus on rehabilitation
  •  
  • Assess and Restore Services Hospital-based outpatient clinic and outreach services for geriatric patients, or individuals under 65 years (case by case basis), who present with geriatric conditions/symptoms.
  •  
  • Falls Prevention Clinic Falls prevention classes for older adults and people with mobility issues or chronic conditions * aims to improve strength and balance to prevent injury and falls * services include physiotherapy and occupational therapy
  •  
  • Orthopaedic and Amputee Clinic Hospital-based outpatient rehabilitation for people who have acute musculoskeletal injuries and amputation * services include pain management, individual and group physiotherapy, and occupational therapy

  • Stroke and Neuro Clinic (Progression and Maintenance) Hospital-based outpatient rehabilitation program for people who recently have had a stroke or have neurological conditions * services include:
