Adults 65 and older * discharged from physiotherapy * have a barrier to a group exercise class * have an Assessment Urgency Algorithm (AUA) between 2-5 * must have functional goals that can be addressed through standardized exercise with hopes of transitioning to group exercise classes

Description of Services:

One-on-one in-home gently exercise program for clinically frail, home-bound older adults * aims to maintain and improve their fitness and independence, to increase the strength and balance, and to reduce the risk of falling * instructor or volunteer facilitates a standardized program of up to 15 gentle exercises that can be modified for all levels of ability * the goal of the program is for the participant to progress enough to be able to join the group exercise program upon completion of the in-home program

Program criteria:

client who have received recent physiotherapy specific to mobility declines will be excluded
client must have functional goals that can be addressed through standard exercise
client must be unable to attend community group exercise classes
client's goal must be to transition to group exercise or continue independently with exercises
