Eligibility / Target Population
Older adults and adults 21 or older with acquired physical disabilities, who need physical, social, or emotional supports * Ontario Health atHome Care Coordinators assess the needs of applicants * must be living in the Mississauga/Halton area * program is not appropriate for people with a diagnosis of Alzheimer's disease or other dementias
Fees
Registration is required with fees calculated on a per activity basis. Activity fees are calculated each session, which varies in length * additional fee for transportation
