Description of Services:
A partnership between Woodgreen, LOFT, Reconnect, South Riverdale Commnity Health Centre and Haven Toronto * trained professionals providing crisis assessment, intervention and support * 6-8 weeks of case management and counseling support to deal with crisis * advocacy, service navigation, harm reduction, health assessments and linkages to primary care through dedicated Nurse Practitioners and behaviour support through behaviour consultants
