Comprehensive assessment by internal consult team * provide recommendations on older patients at Sunnybrook who have complex medical and functional issues * team composed of a geriatrician, nurses, occupational therapist, physiotherapist, recreation therapist, social worker, speech-language pathologist, and volunteers
