Application

Referral from a physician or nurse practitioner is required for many programs and should be initiated through our Central Navigation.

Eligibility / Target Population

Adults who live in the community, at Baycrest, other long-term care homes and assisted living residences in Ontario. Appointments may be in-person or virtual at the discretion of the care team in collaboration with the patient.

Description of Services:

Provide assessment and treatment programs for people suffering disturbances in cognitive function and behaviour due to brain dysfunction * Problems may include memory loss, speech and language disorders, as well as cognitive deficits associated psychiatric disorders such as depression * Our services include:

Assessment and diagnosis
Pharmacotherapy
Non-pharmacological management
Counselling Education

Meetings:

The Clinic provides both in-person and virtual visits at the discretion of the care team in collaboration with the patient. For patients opting for virtual visits, the patient must be physically located in the province of Ontario at the time of the clinical virtual visit.
