Description of Services:
Diabetes education and management support by dieticians, nurses, endocrinologists and a social worker for people living with prediabetes or diabetes including:
  • gestational diabetes
  • pre-existing diabetes with pregnancy
  • post-gestational diabetes
  • type 1
  • type 2
  • insulin pump therapy
  • blood sugar monitoring (including Libre and Dexcom)
  • inpatients with diabetes
