Description of Services:
120-bed complex continuing care centre (73 inpatient beds providing specialized care for patients who require complex continuing care including end-of-life and palliative services and 47-bed unit providing Activation services, which are also called Slow Stream Rehabilitation) * consists of 21 interim long-term care beds (residential care for hospital patients waiting for long-stay admissions) * 24-hour nursing and personal care * access to a doctor and other health professionals

Physician Care - Each nursing unit is staffed by two physicians from the Department of Geriatric Medicine at Trillium Health Partners * each patient/resident is assigned a physician responsible for coordinating the care plan with the interdisciplinary team * physician on call on a 24-hour basis

Nursing Care - Registered nurses, registered practical nurses and personal support workers staff each unit * additional medical support, lab and diagnostic services, and specialty consultations available at Queensway and Mississauga sites of the Trillium Health Partners clinics

Rehabilitation Services - Physiotherapy and rehabilitation team includes an occupational therapist and speech pathologist that work with patients to:
  • improve and maintain functional independence and physical performance
  • prevent and manage pain, physical impairments, liabilities and limits to participation
  • promote fitness, health and wellness
  • provide assessments for appropriate assistive devices
