Older adults and adults (18 years and older) with disabilities, who need physical, social, or emotional supports * Ontario Health atHome Care Coordinators assess the needs of applicants

Fees

$24 per day * fee assistance available for eligible participants * additional fee for transportation

Description of Services:

Adult Day Program (known as Clubs of Acclaim Health) * social, recreational, and therapeutic activities for older adults, frail elderly and people with disabilities in a group setting for part of the day * helps to support family caregivers who provide day-to-day care * assistance with eating, walking, toileting and taking medications (no injections) * transportation is not included in program fee * virtual club adult day programming also available

Participants may participate in any of the following activities:

music sessions or music therapy
sport games and exercise classes
pet therapy
baking
gardening
discussion groups and word games
special events and themed days
guest speakers and entertainers
coffee and conversation
relaxation and quiet time
applied social research projects through the Centre for Elder Research at Sheridan
