Description of Services:

Family practice medical services with healthcare providers multidisciplinary health care teams that include family physicians, nurse practitioners and registered nurses

Services include

one-on-one appointment for individual client
Virtual services
group services with specific themes and objectives for clients and community members (pre-registration is usually required)
diabetes education program
special health education activities and programs for clients and community members (pre-registration may be required)
pregnancy care for women up to 32 weeks
immunization for babies and young children
