Provides assistance navigating the healthcare and social service systems while providing support when facing life challenges and/or changes such as:
  • difficulty coping with memory loss/dementia
  • mental health and substance use concerns
  • staying independent after significant physical decline
  • risk of homelessness
  • elder abuse
