Care to people with complex needs * help to stabilize immediate concerns, improves overall well-being, and strengthens connections to ongoing health and community supports
- wraparound, community-based support for individuals and families with complex needs
- focus on early intervention, prevention, and stabilization
- case management and coordinated service planning
- access to housing, income, mental health, and family supports
- trauma-informed, client-centred, and culturally responsive approach
- strengthens long-term stability and reduces risk of crisis
