Primary care services for patients and families affected by sickle cell disease

Services include:

  • screening for sickle cell trait and sickle cell disease
  • routine immunization for people living with Sickle Cell Disease
  • case/system navigation
  • mental health supports
  • youth support
  • nutrition/food security
  • community outreach and education
