Description of Services:
Private home care agency providing the following services in hospitals, rehab, retirement residences, long-term care facilities or at home:
Five free one-hour visits from five care companions trial
30-minute free consultation and free in-person assessment visit
- companionship, interactive activities
- personal care (bathing, dressing, grooming)
- assistance with activities of daily living (ADLs)
- medication reminders
- meal preparation
- nutritional support
- assistance with grocery shopping and errands
- light housekeeping and home management
- post-hospitalization care
- respite care for family caregivers
- transportation accompaniment to and from appointments
- Alzheimer's and dementia care
- 24-hour live-in care
