Description of Services:
312 beds (total) * residential care for 204 private rooms including 5 bariatric beds and 24-bed Behavioural Support Unit (secure) * semi-private rooms (108 beds)
Additional services (fees may apply) and facilities:
- bedding and furniture
- laundry and housekeeping
- meals
- personal hygiene supplies
- medical supplies and equipment (such as catheters, ostomy bags, or wound care supplies)
- assistance with medication and activities of daily living
- recreation activities, group exercise and social programs
Additional services (fees may apply) and facilities:
- on-site primary medical care
- 24/7 on-call medical care
- dialysis centre operated by Trillium Health Partners (THP)
- geriatric psychiatry outreach
- rehabilitation and restorative care
- dietetic services (assessment, nutrition, counselling and monitoring)
- physiotherapy services
- occupational therapy services
- central therapy
- spiritual care services
- basic foot care
- virtual Long Term Care hub to connect the homes' primary care model to hospital specialists
