Description of Services:
Clinic provides quick assessment and access to diagnostic services and treatment for patients at high risk of suffering a stroke * aims to reduce the occurrences of stroke through:
- early assessment and treatment
- access to specialist and diagnostic services
- patient and health care provider education on stroke prevention
- support and risk factor screening for spouses and family members of stroke patients
- medical care and lifestyle changes
- follow up with stroke patients after discharge
- care team includes neurologist, registered nurse, cardiovascular (CVD) technician, and stroke navigator
