Description of Services:

Family practice medical services involve a multidisciplinary health care teams that include family physicians, nurse practitioners and registered nurses

Services including

one-on-one medical appointment for individual client
well baby visit
immunization for infants and young children
adult health maintenance and management of chronic diseases
pregnancy care for women who are up to 27 weeks pregnant
counselling for mental health and substance use disorders
chronic disease management- Diabetes
nutrition counselling
special health education activities and programs for clients and community members (pre-registration may be required)
case management and system navigation
naturopath
chiropractor
