Description of Services:
Four to six-week funded individualized multidisciplinary program * exercise and falls prevention program for older adults with multiple medical conditions * aims to restore their functional abilities, improve their quality of life, and allow them to live independently or with the support of a caregiver in a community setting * online/virtual available in special circumstances

- physiotherapy
- occupational therapy
- speech language therapy
- recent episode of acute care (hospital admission)
- suffering from stroke, Parkinson's or other neurological disorders
- unsteady on feet, loss of balance, strength, range of motion
- difficulties walking
- difficulty finding words
- coughing while eating or taking longer to eat meals
- gradual or sudden changes to communication, e.g., voice, comprehension
- has difficulty or is unable to perform the activities of daily living
