Description of Services:
Health centre focused on family and community health * health
professionals assess and treat non-life-threatening injuries or
illnesses * referrals to other local health services and personal
development groups
Community-led health teams develop programs to improve the social supports and conditions that affect long-term health
Community-led health teams develop programs to improve the social supports and conditions that affect long-term health
- Diabetes Education Program Offers free individual counselling and group education classes for people with Type 2 diabetes and pre-diabetes * program also offers free Teleophthalmology services including free eye exams and retinal screening services for individuals with Type 2 Diabetes * diabetes educators include registered nurses and registered dieticians
- Drop-In Legal Clinic Provide free legal advice in the following areas of law: social assistance, housing, basic employment, human rights, CPP and EI * notarization of documents (documents must already be prepared)
- Harm Reduction Drop-In For safer drug use kits/supplies, safer sex supplies, naloxone kits, education, referral and support
- Home-Based Primary Care Team Dedicated inter-professional primary care team supporting complex homebound clients residing in the west end of Toronto * team comprised of an Ontario Health atHome care coordinator, physician, nurse practitioner and pharmacist * provides medical care in the home to clients living in designated catchment area and who cannot get out regularly to see a family doctor * this west-based Toronto team will work together with clients and families to help manage a client’s health care needs
- LGBTQ2+ Support A range of free counselling and support services for lesbian, gay, bisexual, trans, two-spirited, intersex, queer and questioning individuals
- Primary Care Clinic Family physicians and nurse practitioners provide primary care to clients
