Description of Services:

Support for those living with a life-limiting illness:

develop and evolve a personalized care plan with you
work with other healthcare providers
provide in-home social visits from specially trained volunteers
help navigate the healthcare system and assist with referrals to other services
participate in individual counselling
support you in having those difficult family conversations (e.g. advanced care planning, levels of care, palliation, etc)
participate in a weekly day program and social club
participation in wellness activities (e.g. yoga, reiki, etc)
