Description of Services:
Assist Indigenous older adults to maintain their health, well-being and independence in their respective homes
Services and Programs:
- medical transportation
- diabetes support group coordination
- personal support worker services
- shopping club
- security and reassurance checks
- social activities
- advocacy measures
- assistance with ADL – Activities of Daily Living
- palliative care supports
- congregate dining
- In-Home Personal Support Assists with the activities of daily living, including personal care, meal preparation, laundry and light housekeeping
- Transportation Non-emergency transportation to local and long-distance medical appointments and regular activities such as grocery shopping and banking * volunteer drivers
