Description of Services:
Care coordinators provide free in-home or in-office assessment at a time that is convenient * help plan care and discuss available options

Program includes:
  • information and referral to appropriate services and resources for both older adults and caregiver
  • assistance with filling out forms
  • crisis intervention
  • assessments to access older adult's needs and provide appropriate referrals for internal and external services
