Home visit and assessment
Eligibility / Target Population
Older Adults 55 years and older
Individuals who are experiencing social isolation and loneliness
Live alone in their own home/apartment
Have little to no family support
Medically stable (does not require extensive medical intervention and/or not in a state of rapid decline)
Physically independent (with or without mobility aids) if social outings are desired
Residency Requirements: Residency Requirement: Don Valley Parkway/Bayview Ave (West), Woodbine Ave (East), Eglinton Ave (North), Lakeshore Blvd (South)
