Eligibility / Target Population
Adults 65 years and older (55 years and older in special circumstances) who will benefit from two or more program disciplines * must be able and willing to participate in a group setting * must be able to toilet independently * must be able to learn and fellow recommendations delivered in English, or be accompanied by someone who can translate * must have ongoing and reliable transportation to and from program

Description of Services:
Hospital-based outpatient day program with a focus on geriatric rehabilitation * services include:
Treatment is offered in three-hour sessions twice a week for 12 weeks
- discharge planning
- individual and group exercises
- health education
- medical and nursing assessments
- referrals to Baycrest services
- social work
- speech language pathology
- physiotherapy
- occupational therapy
