Description of Services:
Specialized community connection support within Toronto and Peel Region focusing on two age categories: elder connection (55-year-old and older) and adult connection (54-year-old and under) * services include:
Individual Service (one-on-one): matches individuals who have a developmental disability with support workers to engage in recreational and educational activities in a one-on-one program * support workers also work on daily skills (such as banking, budgeting, menu planning, meal preparation, grocery shopping, household management, community safety, and etc.)
Networking Service: matches individuals who have a developmental disability in a group setting to engage in recreational activities * individuals will be given the opportunity to learn important social, communication, cultural, educational and vocational skills that they can transition to community experiences
