Eligibility / Target Population
Adults 16 years and older with severe physical or multiple disabilities, acquired brain injury, and high care needs (such as multiple sclerosis, cerebral palsy) who need assistance and support in daily living activities

Description of Services:
Assisted living supports for individuals living in their own homes * assistance is offered up to 24 hours per day
- personal grooming and hygiene
- taking medication
- essential communication services
- for existing clients, palliative support can be provided when appropriate
- meal preparation
- rising and retiring
- dressing, undressing, bathing, washing
- toileting and bowel / bladder services
